Grant Report Guidelines

The directors and staff of the Elmina B. Sewall Foundation maintain a continued interest in the organizations and projects we support. Therefore, we request that all grantees submit a grant report in accordance with the terms of their grant. In most cases, this means submitting a report upon completion of the project or 13 months from the date of the grant award.

Grant Report Form (for grants awarded prior to 2011 only)

The Grant Report Form can be downloaded below. The form and instructions vary depending on whether the grant is awarded for restricted or unrestricted purposes. Grantees should be sure to select the appropriate form when submitting their report. Should you have any questions please contact Foundation staff.

Grant Report Form (for grants awarded in 2011)

The Grant Report Form is available through the Foundation's Online Grant System. You may access your organization's account by pressing the "My Account" button below. The link to your report was also emailed to you shortly after your grant was awarded.
